Best example has to be Parma. Won different cups in the 90's incuding the UEFA Cup but now play in the fourth division in italy due to bankrupcy and bad management. Nottingham Forest(not in top division but former European champion), Athletic Bilbao, AC Milan, (although not really struggling to stay top, they are a shadow of what they once were), Rangers(50+ Scottish champions) are now in the second division. Liverpool (5x European champion) are highly inconsistent
