If you mean the plastic cases.. it's because people vomit, bleed, pee and poop in the hospital and sometimes it can end up on the bed or pillow. The plastic makes it possible to wipe and clean off. If it was cotton... well, that would have to be washed with bleach and dried and they have no time for that. Lol
