Where does the balance of perscription drugs go?

If you have insurance and a $400 perscription costs $45, where does the balance go? Does the pharmacy get a cut, the drug maker obviously and do they zero out the difference?

    Your insurance negotiates a price with the pharmacy. You pay $45 of the prescription, your insurance company may pay about $250 or so - depending upon what the prescription is. For some it may be more, others less.
