What documents & forms does a retired veteran's widow need to renew dependent card?

The veteran served two tours in VietNam and 20+ years in the reserves. He also served 30 years in federal law enforcement. He passed away at age 61 and had started receiving his military retirement pay. The widow has some mobility issues & is living off of his death benfits/retirement pay. She needs to renew her military dependent card, which expired several years ago, but does not live near a base and does not know what is needed to do so. What information does she need to provide to get her card renewed, and what forms does she need to fill out?

I forgot to mention that the veteran served in the army, if that makes any difference.

I was trying to determine which form and what status. I think she would be in "Ret-blue" status since her husband was receiving retirement pay.

Ok, Thanks to Daniel, I called the nearest place-- phone rang 30 times before they picked up-- and asked. I was told that she would need her old ID (did not matter that it was issued before her husband passed away) and a photo ID. So basically, her old ID and her drivers license. Now I just need to see if my phone's gps can track the location well enough.

    There are a lot of locations other than military bases that issue ID cards (like National Guard armories).

    Once you find a location, they have contact info. You can call them to find out exactly what she needs.
