Just start paying them what you can afford. Send a letter with the payment stating you will pay this amount per month.

You have no particular option here. You can bargain them down, but it costs what it costs. $90/month means picking up a few hours a week part time at some crappy job. You can clearly do that, so you're not going to get much sympathy from them on that.
In the future, ensure you have health insurance and vote for politicians who believe in single payer non profit public insurance. In Canada, this would cost you nothing. I had a friend who damn near cut his thumb off on broken glass, and had far more stitches and a surgical consult to ensure there wouldn't be long term damage. It's all covered under public insurance. No copay, deductible, etc. That kind of peace of mind is priceless.
Also don't run to the ER for a small cut involving two stitches - a walk-in clinic or family doctor can handle that. Or just natural healing can.

The party who can "help you out" can be seen in any mirror. You went to the ER - that's why the charges are so high. Too bad you didn't use an urgent care clinic instead, not to mention have insurance. Take the payment plan. If you can't even afford $90/mo, as you claim, then your financial problem are much deeper than just this bill.
