How do I calculate insurance reimbursement?

There aren't any examples in my textbook -___- I'm not gonna write out the entire question just the important parts.

The physicians fee is $75

The patients copayment is $20 and is not required to pay any additional amount to the provider. The payer reimburses the physician $28. How much does the patient pay the provider? How much does the provider write off? Do I take the amount that is reimbursed and subtract it from the physicians fee? OR do I add together the copay and reimbursement to then subtract THAT from the physicians fee?

    The patient pays the copay "and is not required to pay any additional". So what does that mean?

    So fee - (copay + reimbursement) = write off
