I'm using Adobe Photoshop CS3 and want to upgrade to CS5. Do I need to purchase the extended edition/version?

Update:

I realize Adobe is going towards subscriptions with CC (Creative Cloud) but I can't afford to pay monthly as I live on social security.

You can't buy CS5 directly from Adobe. For one, it has been discontinued. For two, Adobe ONLY sells their Photoshop products via monthly subscription now.

Be very careful getting CS5 from a lot of these website claiming it's "free" or "cheap". Those "free" and "cheap" pricetags come with virus packed programs.

I know Amazon.com still sells CS5. But it's VERY expensive.

Since you are using CS3, I assume you have an old computer. Make sure your computer is capable of running a newer version before you jump into an upgrade. If it were me, I'd just stick with CS3 and not waste money on an upgrade. Is there something in the CS5 version you think you need over the CS3? I didn't see anything in the CS5 version that I needed over the CS3 and thus didn't find it to be worth the money.
