shotgun formation 5 yards behind LOS. Center snaps the ball. Ball is in the air, can the qb step up and get the ball at the 4 yard line?

Yes. He doesn't have to wait for the ball to gt to him. Unless the play is a run it would defeat the whole purpose for using the shotgun but it's legal.

Yes, as soon as the ball is snapped everyone can move forward if they need to.

Yes but the snap is so fast that the qb would have to time it perfectly.
