    Do NOT use a commercial website! You can ONLY go to an official US govt website. The US consulate website for the consulate in your country may have the ESTA form, or go to travel/state/gov, etc. Door gave you website addresses, so I won't repeat them all.

    As for which airline to choose, book online with any carrier you choose. E.g. you can go to kayak, Travelocity, or many other websites to compare airfares, schedules, etc. If you are a member of an airline travel club to get points, free upgrades or other airline benefits, check with yours to see which airlines belong. You'd probably want to use one of those member airlines. If the price is right.
