I guess the only way you'd know is if you see activity on the account that you know you didn't do, like a read message. In the case of Gmail it usually states the last time the account was accessed & I think it also gives a general location as well.
If you think that is the case changing the password would help of course.

Do you have any reason to believe your email account has been hacked? Either way, it's a good idea to change your password every so often.
