He is sick, he has diabetes. If he took blood glucose readings after eating all this junk, it would probably be through the roof

And every time our blood glucose goes above 140 we are damaging organs and blood vessels.

Because the kinds of problems that eating sugar does to a diabetic can't be seen right away.

Right now, he probably just has issues with peeing often and maybe headaches and lack of energy. (dehydration and drinking a lot are probably also common for him)

Over time, the continued high sugar levels will do damage to his blood vessels and his nerves. He will develop diabetic neuropathy which will cause pain in his feet and legs. Besides the nerve pain in the legs and the feet, he will also lose the ability to feel real pain in his feet. You will need to check his feet often to make sure he doesn't have any cuts or injuries that he hasn't felt or noticed. Cuts and injuries to the feet of a diabetic that go untreated is one of the main issues that might end up causing the need for amputation of a foot. He could also develop issues with his vision and would be at risk for going blind. Over time, his kidneys will also begin to fail due to continued high sugar levels.

Everything about diabetes is a slow process. It isn't something that suddenly develops. Depending on exactly how high he is allowing his sugar levels to stay, the things I mentioned could take a few years to develop.

By the way, sugar levels that go over 500 can create a risk for diabetic coma or diabetic ketoacidosis. Diabetic ketoacidosis is more common in type 1 diabetics but could happen to a type 2 that has very poor management of their condition. Either of these two things could happen fairly quickly and could turn deadly. You may want to do some research for the warning signs of these two conditions if he is going to continue to eat poorly and poorly manage his sugar levels.
