How do I switch my doctors office?

Hey I just moved to Chester PA from wilmington de and when I was in wilmington I ve had the same doctors office never switched since 2011 so now I need to go about switching to a doctor s office in chester?

    just make an appt and tell the other one you needed to switch cause of moving

    You can request that your records be sent to you, or to your new doctor.

    Or the new doctor can request your records from your previous doctor.

    You will probably need to provide written permission for the records transfer.

    Call the new doctor's office and ask them.
