Will it be legally valid if I say in a misconduct report that "I hereby wish to report Name Surname and I do not permit disclosing my name"?

Update:

... "and I do not permit disclosing my name to the person who I reported.". If I write that and if the supervisor still discloses my name, can I take a legal action in this case?

Report to whom? Reporting what? Legally valid under the laws of what state or nation?

Your statement carries no legal weight.

Could you be protected under some other law? Maybe, but that wasn't your question.

No, that doesn't stop them from reporting your name.

You know, the whistleblower laws apply equally to everyone, right?

If you're going to whistleblow, there is a form where you not only say what you saw (it's supposed to be firsthand knowledge), but you have to identify yourself. Schiff changed to form a month before the whistleblower came forward, to read that second hand or hearsay knowledge was acceptable. These people are despicable!
