not all people who smoke real cigarettes get cancer so the answer to your question is no

There is not a lot of statistics and studies being done on herbal cigarettes including marijuana. But it's not the nicotine that gives you cancer. You can take nicotine everyday of your life. And probably never get cancer from it. Bitter greens like collards and mustards have nicotine in them and they never give you cancer.

It's that most smoke contains tar and other irritants that repeated the injure the lungs. When you repeatedly injure a place in your body your body will develop either scar tissue or it could go to cancer.
