“Monkey on the back” is an old phrase that refers to a habit or problem that is difficult to get rid of. Drug addiction would be ripe for that analogy.

Nothing to do with black magic. People with a drug problem are said to have a monkey on their backs. It probably comes from the fact that if a monkey jumps on you, it's really hard to get them off. I know people who have had monkey jump on then in China. They couldn't shake the monkey loose- just like a drug addicts can't shake the habit.
