What mental issue can cause someone to think everyone who talks to them has an ulterior motive?

    Paranoia.

    It's obvious.

    This would be "paranoia" which can be "good" (e.g. this person worships me) or "bad" (e.g. this person wants to kill me), so;

    Schizophrenia Spectrum; Mostly the "bad" kind. Could be specifically "paranoid schizophrenia" though not necessarily. 

    Personality Disorders: A range of PD's can include these thoughts. A Histrionic PD will tend to have "good" whilst a Borderline PD may have either and a Paranoid PD would have mostly bad.

    Bipolar Disorders: "Good" during mania, "bad" during a depressive.PTSD Disorders: "Bad".

    Depression & Anxiety Disorders: "Bad".

    I say "bad" and "good" loosely - No amount of delusional or paranoid thinking is good, but these thoughts can revolve around many different things, either based in reality or not.
