What would be the most likely cause for Insufficient coolant temp for closed loop fuel control?

1998 Mitsubishi Galant - The original code was p0170 - Fuel trim malfunction, so I replaced the gas cap and added a can of B-12 to the tank to clean the injectors and erased the codes. I then drove the car about 85 miles. The check engine light came back on, code is p0125 - Insufficient coolant temp for closed loop fuel control. I then replaced the thermostat, erased codes, and made it 97 miles til the light came back on. Code is again p0125. The radiator is clean and coolant good. What should I try next? Any help will be appreciated.

could be a stuck open thermostat id reccomend one from the dealer sometimes aftermarket parts dont perform to factory standards btw what degree thermostat did you put in should be a 195degree

Could be a bad coolant temp sensor or sending unit.

maybe electric radiator fan runs all the time, also maybe faulty coolant temperature sensor.
