Gross pathology are changes that are visible by the naked eye, and microscopic pathology are changes visible by microscopic examination. Usually the gross appearance is used to identify what to sample for microscopic examination (microscopic pathology - if you will). For instance gross pathology will tell you that a lump (which might be cancer) is present, but to know if it is a cancer (sometimes this is evident from the gross appearance) and what kind of cancer it is, you need to do microscopic pathology. The surgeon also uses gross pathology to decide what and how much to remove.

Gross pathology is what you see and microscopic pathology is what is seen under a microscope.
