The anime ends at chapter 60, so read it from chapter 61 and onward if you want to continue where the anime left off.
The manga also includes another character not introduced in the anime named Nozomu.

The best sites I can find are listed in the sources section. Just follow the links provided and you should be ready to read away. Btw, be ready to cry, this series is amazingly sad. The site you choose should be based on your personal preference. I like mangareader.net better because it is simpler, thus it loads the images faster. I find their translations to be more accurate as well.
