Any United Health Care managers out there?

What happens when the 100 days of hospital/skilled nursing runs out? My 93 yr old mom has been in skilled nursing for quite awhile and her 100 days runs out next month. So what happens now? She has the best AARP/United Health care Plan D. Thank you
