When in the surgical residency process do you choose your specialty?

After year one which is gen medicine. Some sub specialties are a fellowship after a gen surg residency, others are specific surgical residencies like ortho.

It's more of a calling from what I gather talking to surgeons. It chooses you otherwise everyone would be in plastics.
