This was probably a lot funnier 20+ years ago when almost everyone would hear the correct answer of 'mother' and be surprised, even shocked, that they didn't think of it themselves. Back then it was more unlikely, although very possible, to hear of a female head surgeon, so the correct answer was more unexpected.
