Why does my "safely remove hardware" icon disappear from the system tray?
And what can I do to safely remove my USB device when the icon disappears? (I've tried "eject" and I get an error.)

I think you may have the problem with your USB port. Just plug the device in other USB port. If you get the same message just go to the(Assuming that you have installed win XP)
My Computer>>Properties>>Device Manager>>Universal Serial Bus Controller(Select it) and uninstall all the ports and reinstall.
In win98 My Computer>>Hardware>>Device Manager.
Same procedure.
Just see what happens.
