I have had to reload once in the past 2 years, due to a hidden proxy server which I could not eliminate.
I always reformat a full HDD whenever reloading an OS.
I believe most crashes are caused by user inexperience, visiting websites that are destructive or having destructive links associated with them, or by not keeping their PC properly protected from viruses and other intrusive/unseen programs.
I believe the second largest cause of PC crashes is defective or failed hardware.

My computer crashes About once a month.
Because I didn't shut it down for 5 days...
I re-install about once a year.
I've never format my HDD since the first time. XD
May be you'll have to do like me. Dropping Windows trought you window and grabbing anywhere a Linux distro :p

Never...
If your crashes often. You may have viruses or spyware. You can get Norton Antivirus, McAfee VirusScan to remove them. Use Adaware to remove spyware.
The best way to prevent this is to have this software utilities install and runnning all the time on your computer (specially when you are online).

2 or 3 times per year just because i can
crashes that often are a signal that something's seriously wrong with your PC. I would have someone check it out to determine if it's hardware(Hard drive may have bad sectors) or software related.

None of the above have happened. If you're experiencing frequent crashes I'd check for viruses.

Twice, it was the sound card I forgot to update the driver and since the driver update no crashes.
