It's a tradition to exchange jerseys with an opponent you admire or respect. Players go up to a member of the opposing team and ask them if they're willing to trade. Most often players are open to it. They keep the jersey they get as a souvenir of the game.

It's just to have a momento from the game.Most players keep the shirts and display them in their personal trophy rooms.
