if a technician installed a video card into a computer with an urgency of the upgrade in a carpeted office and the person using the computer then reported that the video card only works intermittently what is the most probable cause of this problem?

worst case scenario is that the carpet caused ESD: an ElectroStatic Discharge to the videocard, thus damaging it.
But since the card only works intermittently, it's more likely the card wasn't installed properly. So either the drivers are wrong, the cable is poorly connected to either the monitor or videocard, or the videocard isn't seated properly in the slot.
To be certain, swap the monitor as well: maybe the monitor is simply at it's lifes end :)

the correct drivers were not installed, the card isnt seated properly, the card is not compatible with the operating system of that pc, the cable from the monitor to the video card is loose,
what does a carpeted office have to do with anything?
