When a pitcher is pulled....?
what happens if a manager makes a motion to the bullpen for a relief pitcher and the wrong pitcher comes out? Can he tell the umpire the wrong pitcher came out, get the pitcher he wanted and still have the one that came out by mistake stay in the game?

The key aspect of this senario is if the pitcher has been officially announced. If he is then he is the pitcher of record and needs to throw a pitch. If not, the manager can motion to him to return to the bullpen. I have seen this at the high school level but never in the majors.

A relief (substitute) pitcher is not legally in the game until he has taken his place on the pitchers plate. Up until that happens, the manager can adjust the line-up.

It depends. As long as the correct pitcher is brought in before the mistaken one throws any pitches. It's not a matter of being on the field. It's a matter of playing.

No. Once a pitcher enters the game, he cannot be removed and still be eligible to re-enter.
The manager is a dumbass who needs to have better communication with his bullpen.

The umpire doesn't register a change based on who comes onto the field; it's done by way of the manager or one of his staff telling the umpire who the replacement is.
