Spaghetti and meatballs would be correct.
Spaghetti with meatballs implies that spaghetti is your main dish, meatballs being the side dish.

Spaghetti and meatballs. Or if you go to Tomato Brothers, spaghetti and meatball. They give you a baseball-sized meatball, it's insane.

Are you cooking the meatballs in the gravy (sauce)? Then it's spaghetti AND meatballs.
AND is defined as added to or plus.
Are you cooking the meatballs separately and serving them as a side dish? Then it's spaghetti WITH meatballs.
WITH is defined as accompanied by or accompanying

most menus i see says AND. even canned food!
spaghetti meatballs sounds like the meatballs are made out of spaghettis. i suppose you can say that to a lunch room server in school though - they'd prolly understand.
