Someone emails you from Yahoo Answers and you write back to them only to have it fail because their email was not confirmed .
It is frustrating !

Yes. usually caused by hitting a wrong key when writting the send to address. Check to make sure the person is accepting e-mails should be in their profile page.Or try resending as the e-mail does not always work the first time.
As if there isn't enough minor errors to drive you up the wall.

Yes it has happened to me. And it is frustrating specially after you were inspired to write them in a way. Why?
