Why does defrag Fail?

I really need a tech to help with this. I asked once and got no decent answer. Anyways, the previous question was( and I am editing and adding bits):

I run defrag with an /f switch,as well as with /r switch, and also without the switch ; I also run it from the command prompt. Its not the "Neat Freak" in me, since I know that a 0,0,0 defrag value set is possible ( the majority of the machines I do achieve that); its the one that fail that have me stumped. There is a set of commands like chkntfs or similar, and the command allows defrag to run fine. These extra command appear to unlock some drives.Also, some hardware settings can cause the failure, I just can't figure out how to find which is which.

Totally stumped... I have tried so many permutations. I have been working on several hundred computers over the last few months, and I need to get as much info as possible. Almost all of the PCs are running XP Pro SP2. I really need an answer from someone with a networked PC world...

No, all the PCs have more than enough free space.

There are no other apps running at the same time.

Safe mode has also been tried, still I have the same issues.

Well, yes I am kind of assuming Hard drive issues, but I have tried all varieties of fixes on hundreds of machines; its not like I do not know what I am doing, I just need some help, since I can't figure out what to change, and I am doing this on many, MANY machines

    you might have hard drive problem or you use you computer when you run Dfragment

    I used to have problems with defrag. It would get to about 10 percent and stop everytime.

    A while back on some question site (not this one), someone told me to restart the computer and hit F8 during startup. Then select safe mode. Defrag from there. Well, I tried it and it worked fine.

    It may be failing due to the lack of available disk space. Windows XP prefers to have atleast 15% disk space available to properly defrag I believe.

    The cause for this could be another program that is running simultaneously. Try closing all other programs, and then defragment.
