What forms do I have to complete to ship something from US to canada?

I have a $25 item that I want to ship to Canada through UPS, and I need to know if there is any customs forms to fill out to ship it.

    They will have them at UPS. They're simple declaration forms (ie. no dangerous goods, nothing for resale etc.)

    ups has the forms declare as a gift

    depends what the item is, not what it costs. see if you can have the recipient declare it as a gift. if you are the reciever, use another name/address as the return address.
