Does anybody know where this phrase comes from, and can you give me some examples or stories that can let me understand this phrase better?

Well. If you're going to go sky diving, you had better be positive that your parachute is correctly packed and all in order. In other words, you had better do it right the first time. You won't have a second chance!
Despite the saying, "If at first you don't succeed, try, try again," for some things in life there are no second chances.
