Make him dinner. MAKE him dinner. Don't throw a Lean Cuisine in the microwave. Put some time and effort into it. Guys don't care to get material gifts from women just show him you care by spending a little bit of your time shopping and preparing and cooking. If it's not good enough for him and he wanted an X Box game instead tell him he;s a douche and find yourself someone better.

I think the most important consideration is to get something that will help both of you remember this occasion forever (for better or worse!).

Hence, get something durable and visible. You haven't specified a price range, but I reckon you could try a watch, or a piece of jewelry. If you're creative, a self-made gift or card can go a long way.

Don't get something you'll eat and forget, or flowers that fade.

shopping in the ports or on ship will be fun...find something there, not before the cruise. OH OH wait I got it...have the room decorated for his birthday, the ship will do for a price.

Well, you're with him on a boat, sharing a room I immagine. I am sure you can come up with something.

get him one of those visa gift cards so he can spend it wherever he wants. also if u know his favorite band get him a t-shirt. that works too
