can you go swimming at navy pier?

No you can't. But there is a little area just north of the water intake pier where you can go. It's inside the rock jetty and has rope floaty-like things out in the water to show where you can and can't swim. By the way, the water is probably nice and cool and I sure miss it!

Not technically AT/ON the pier, but just north of it (really, JUST north...basically still at the pier, you don't have to go as far as North Ave. beach) there is a beach called Olive Park Beach. It's kinda small but actually a very nice and underrated beach where you can swim. If you're on main street Navy Pier, it's a 3 minute walk. Just exit the pier and go north, it's right there.

A couple years ago a woman was doing that and lost part of her leg when a tour boat hit her. Sounds gross and I am sure it was. There is a beach nearby but technically not on the pier.

No. Definitely not. You can go north a couple blocks and swim at the North St. beach though. Have fun and don't forget the sunscreen.
