All shops have different procedure but basically what u want to watch out 4 is: What age they pierce nipples at - if theyre willing to do a,say,15 year old girl, run!!
Do they use anaesthetic? Ideally ethyl chloride wil be used to freeze the nipple. Do they offer a good aftercare policy, can u come back with any issues or healing problems? Do they use a single use needle? They should show u the pack being opened, dont worry u dont have to look at the needle if u dont want.
Basically the nipple is marked with a pen either side and clamped. This sounds painful but the clamps are tightened with elastic so theyre not so bad and this should really be the most uncomfortable bit. Then area is frozen and pierced and a bcr (ball closure ring) inserted. This is needed just for healing period as a barbell or bar cannot be turned to aid cleaning and can catch and pull (oww!) It can take from 3 weeks to heal onwards depending on the individual butit should be cleaned 3 times a day with saline (salt water) solution which u can buy at chemist or make yourself. Soak cotton bud in saline and thoroughly clean ring and pass it through piercing. If you clean the gunk well off first it wont hurt at all to turn ring. You know youre healed when you've went about 2 wekks with no crusties or gunk coming out!

First. Your piercer should sit you down and explain exactly what they will be doing.
Every piercer will do it a little differently. But should be doing the same things.
With gloved hands.
They should first lay out paper towel. Then lay down either a sterile tray or a backed towelette, which means it doesn't bleed threw liquid.
They should then lay out all there instruments.
2-Alcohol (Single Use Package)
1-Iodine (Single Use Package)
1-Lubricant (Single Use Package or squirted onto tissue from tube)
1-Cork (Still in Sterilization Package)
1-Marking Pen (Single Use Pen or Pencil Stick and Ink)
1-Needle (Still in Sterilization Package)
1-Clamps (Still in Sterilization Package)
4 Sets Of Gloves (always extras so you don't need to hunt if you need more)
1-Piece of Jewelry (Can use barbell, horseshoe or Captive)
3-Folded up Paper Towels
Change Gloves.
The peircer will prepair by starting a tare in the packages so they are easy to get to.
The lubricate will be squeezed out into the towel, put on the tray and the needle slid threw it from the side so it is covered in lubricant.
Change Gloves.
The peircer will now have to remove your top to prepair for peircing. The nipple will need to be erect to be peirced. Usually shops are kinda cool. So with the help of alchole and the climate the nipple will usually become erect.
The peircer will ask you to stand up with your breasts at there natural position before marking where the peircing will be. This insures that they are straight.
After marking the place where the needle will be incerted and come out, the artist will then clean the nipple with iodine and again with acohole.
At this time the clamp will be placed on the nipple. The artist will remove the camp from its package and place it on the nipple. Aligning the markings to insure a straight threw peircing.
You will then be asked to sit down. You will not be peirced standing up because you are more able to move and could be hurt if you were to faint. Which some people do, do.
The peircer will hold the clamp in one hand and the needle in the other, with the tip in the middle of the marked dot.
The peircer will tell you on the count of three he will do the peircing. Most times, they count and right after they say two, they push and by time they say 3 its over.
The peircer will allow the needle to stay threw the nipple while they remove the clamp, there is very little bleeding because the needle itself is blocking the flow of blood. Once the clap is removed a cork will be put on the sharp end of the needle to keep you from poking yourself or the peircer from being poked.
Change Gloves.
When you are ready, they will give you a minute or two to regain yourself. They will take the jewlery out, put lubricant on the jewlery and butt the screw thread of the jewlery to the needle or inside the needle if they used a larger gauge. They will then "chase" the needle threw the hole with the jewlery.
The needle will be discarded into a sharps container.
Change Gloves.
The piercer will now ball your jewlery and clean up the area.
All the equitment should be rolled up togather and thrown in a red trashbag.
The clamps should be wrapped in paper town and sent to the sterilization room.
Your peircer should then sit you down, explain how you will need to take care of your peircing. Answer any questions and give you a contact number if you have a emergancy.
You should be told to clean your peircing with antibaterial soap with no color or perfume added. The best way to do this is shower. Once you have been in there long enough that the crust around the nipple is soft clean it off, soap the nipple, and move the jewlery back and forth. This may sting for the first couple days, but it is nessary. And when your done cleaning them they will feel so much better. After 4-5 days, you should use a sea salt soak. You can use Marine salt from your local pet store. Fill a shot glass or small cup with warm water and salt until foggy and hold them to your nipples for about 3 minutes. Clean them 3 times a day. Soaking twice. And they should heal wonderfully. Don't try to change jewlery before healing, you may not be able to get it back in without doing yourself harm.

From what I hear and have read about, they use a clamp on your nipple and then stick the needle horizontally through it. I've heard that if your getting both done, the first one is the worst, and the second one you can't really feel because your still in shock from the first one. It is a really sensitive area, though, so some of the people I've talked to have said it hurts really badly. I think it takes a little longer then other piercings to heal...8 weeks I think. Its always important to go to a professional...I read an article about a girl who did it herself and she ended up getting a blood infection and having to get part of her breast cut off. Its a pretty safe piercing, though, after its completely healed. You'll need to clean it, like you would with other piercings. See what the piercer reccommends and use it faithfully. Once it's healed, you [most likely] won't have any problems with it. The rings you can get for them are cute to!
