Copy all your files to backups, format the drive & do a fresh install of XP. It's the only way to guarantee you'll get all the nasties off. It is possible to use spyware/adware/malware removal progs, to get 99% of the garbage, but you will spend days doing this, and if you aren't tech savvy, you may never know for sure if you got everything. Some really insidious malware can reside in the boot sector of your hard drive, and just keep re-infecting the machine.
A fresh install is safest.

Try running Superantispyware and Spybot S&D as well as your anti-virus product in Safe Mode. To get into Safe Mode, reboot and then either hold down or tap the F8 key repeatedly while the computer is starting up.
If you cannot get it removed after the above, you may want to join a tech group that specializes in providing manual instructions on malware eradication.

If you've already done the AV and spyware removal stuff than you're going to need someone familiar with Windows' guts to rip it out by the roots. If you're in NE TX I'm very familiar with this and available.
