Most will prefer Doctor, as it's the title that took longer to earn. More than one military M.D. has told me "I got my commission as soon as I came in, but I slaved away for years to get through medical school."

Generally you address them in whichever capacity they're working. Some prefer one over the other though.
And Navy Lieutenants are the same grade as Army Captains, just FYI.

If he's still in the army, call them Lieutenant. If they are retired, call them Doctor.

I think it depends where they are at the time. On duty lieutenant..off duty Doctor..maybe.

that makes it easy....
but you could always ask them what they prefer.

if you are in the military...he is Lt. or sir...if you are a civilian...call him whatever you want...probably doctor...
also...depends on what sittuation you are addressing him in...if it is for medical...Dr. trumps Lt....

You might ask which they prefer. They might surprise you and say it's okay to call them by their first name.

There are no doctors who are lieutenats they are all Captains and above.
