Does anyone know where I can find a sample copyright release form for my customers?

I am looking for a sample copyright release form for my customers. I am a photographer and am working on starting up a studio. I am not very good with words so I need a sample that I can use and I can reword things. If anyone knows where I can get one that would be great.

Update:

ok i am not looking for a model release i am looking for a sample copyright release for my cds that my clients can buy from me for a certain price...they need a copyright release to print what they want b/c certain places wont let the do it without my release

Update 2:

The type of release depends on the situation. In all probability you are in search of a model release form. You definitely don't want to release your copyright priviledges. Just keep that in mind. The only time that a model release form is needed is when you iintend to distribute the photos publicily to demonstrate or display your work such as on the internet or in windows, etc. Even when displaying your photos in magazines or newspapers you do need a model release form to protect yourself. I do have one for models that I can share with you but you have to change the wording to fit the sitution.

One good example of a model release is: located at : <http://members.aol.com/kevinoneil/model.html%3E or antoher one is : <http://www.danheller.com/model-release.html%3E and that last one goes into quite some detail about the whys and why nots of model release and when and when not to consult a lawyer. Most of the time its not necessary to do so and the site explains why.

    Model release form:

    Photo release form:

Just type in sample copyright release form and what type you want into any search engine and you will find many more. Good luck.
