When a person speaks, do they say exactly what they intend to. Assuming they do, does the listner interprest what has been said exactly as it was intended.

This failure of communication is the basis of most problems. Following that is anger that comes form misinterpretation or from hurtful comments. In sequence there then comes the exchange of angry words spoken with the full intent of hurting or angering the object of the comment.

I would reword that say that misunderstandings are the main cause of conflict.
