<<Fait>>, not <<fais>> should be used.
<<Se rappeler>> or <<se souvenir de>> are both acceptable. They are synonyms. That is the definition of a synonym.
Also, are you reciting this to one person/multiple people in a formal/informal setting? <<ton>> ou <<votre>>, c'est marche!

The participe passe of "Faire" (to do)
is "fait" and not "fais"

It's okay, but "Nous avoins fais" should be "Nous avons fait."
