What is the tipping procedure in Lisbon ... hotels, cabs, restaurants, etc.?

It's very different from America here. We don't leave 10% of the check like you do. We usually leave about 1€ on average. In restaurants about 2€ is very generous and in cabs 50 cents should do it - the fares are high enough as it is. You will probably overtip in the beginning because you want to do the right thing, but trust me don't leave like 5€ in a restaurant - it'll be kind of ridiculous - unless it's a super mega luxurious restaurant.

In Europe, waiting staff in restaurants and cafes are paid a proper wage, so service charge is usually included in the price, if not they will let you know somehow. A couple of Euros for good service is generally acceptable.
I don't know about taxis, I never take them, I just rely on the trams or buses.

in hotels, about $10 is a lot, for cabs, even $1 is appreciated, restaurants, about $5 is very appreciated
