I am 35 years old, I need my shot record from childhood. Where can I get that from?

I need it to get into nursing school. I need it for the DTP and they need to know if I took it the first time.

That's that diptheria, tetanus, pertusus shot. If your parents are still alive, ask them if they have your childhood immunization records. If not, it's not a big deal, as most immunizations can be tested by a blood test to check your immunity.

You have to have the booster to tetanus & diptheria every 10 years. If you don't know when you last had it, they will usually give you the Td (tetanus diptheria toxoid) series of 3 shots --the first 2 are administered 4 weeks apart and the third shot is given at 6 months. They can also check your anitbody levels. Here is a good website to figure out what you will need.

Mumps can be tested to see if you have antibodies against it, as adults can also get mumps.

Usually doctors keep a record of illnesses and the shots that were given to a patient. Try to recall your pediatrician, or where you were getting your immunizations.
