Step-by-Step: How do you lay a floor?
The entire floor needs to be replaced, and we are arguing over how many layers of sub-floor needs to go down and the thicknesses. It seems a simple process, rip out the old, check out the levelness of the joists, shim where it's uneven, cut the subfloor where it can be nailed down on the joists, then level it out with self leveling compound then install your floor on top. Am I missing some steps or what?

You did not mention the type of flooring going down. If it is hardwood or carpet you only need the 3/4" subfloor as you mention above. However if lino or ceramic is being installed a 1/4" to 3/8" plywood subfloor needs to go over top of the 3/4" on a perpendicular and making sure that no joints match up together.
