all doctors have to complete a period of residency (around 3 years) in a hospital before becoming an actual/ independent doctor and pursuing a fellowship.

after successfully completing residency, the doctor (officially) can chose if he/she wants to be a hospitalist in a hospital (and be on call all the time which is crazy in my opinion), to be a registered doctor in a hospital/ clinic... etc.

in other words, there are many options once a person becomes an actual doctor after completing residency and being a hospitalist is one of the many options.

    hope this helps
