a lot of the times its because keepers or volunteers (which shouldnt have contact with animals like that) are stupid and careless.
they get confident and think its ok to do certain things with animals.

Death is very rare... however injuries are another story. For the most part due to emergency response and animal excape teams, various safety measures and how fast EMS can arrive and treat, well stats have improved greatly.
If you are interested in such cases try checking out the following site. It contains info from all over the world... just keep in mind that the animals involved are both captive and wild.
