Just be honest, so you don't burn any bridges. You want to leave on good terms so you can put this job in your next application. You never want to just quit. It is always best to work a notice. It is usually customary to work a 2 week notice, but I would at least offer a 1 week notice. I prefer talking with people face to face rather than putting a note on a desk or some other impersonal means. One thing I would do is explain why you are leaving.

As awkward as it may feel, leaving a job is another part of life. It's best handled face-to-face, giving your soon-to-be-ex-employer two weeks notice whenever possible. Be honest and upfront - your boss should appreciate it.

I think it's ok just to tell him in person. Generally, one should give a two week notice, but it might be ok not to since you were working so few hours.
