Cant' remember the name of a book?

There was this book I read when I was in elementary school about a young girl who had an uncle or cousin (male family member) who lived with her who was mentally retarded or had downs syndrome. The title had the word Clown in it. In the story I don't think the girl was ashamed of her family member but had to deal with a lot of heckling from peers. I believe at the end of the story the guy dies. Does this ring a bell to anyone or am I the only one who read this book???

Her mom's optimal cancers diagnosis shocks and unnerves her, even nevertheless the teen and her mom take care of the situation with realism and honesty.
