very difficult on a scale of 1 to 10 ?

with 10 being worst, to totally reconfigure the plumbing in a small bathroom which must be gutted to the studs? Such as moving where the toilet will be?

    7-10 depending on the accessibility of the plumbing If your on the 1 st floor w/ access from below. 7-8 , if your on the 2nd floor 8-10 ..maybe higher lol.. GL

    8, but there are too many variables to be sure.

    First what floor is this on?

    Second how skilled are you?

    Third how extensive are the changes?

    Moving a second floor toilet is much easier than moving one on the first floor, unless you have a basement or a crawl way under the first floor. In that case it is easier, much easier. But, if you have a floating foundation then you will have to tear up the concrete foundation to reposition the toilet piping.

    The same considerations have to be made for the water lines supplying the tub/shower and sink. The supply of hot and cold water is not the problem the problem is the drainage you have to have gravity work for you, you need the loop to prevent the flow of methane into your house and you need to pitch the waste piping so that it flows out and to the street and doesn't lodge inside the household pipes.

    Replacing existing fixtures is fairly easy say 4, if you strip the wall to the studs then redoing the hot and cold water plumbing is very easy 2-3. Moving the waste can be from 2-10 with 10-12 being where you have to dig through the foundation. Frankly, it might just be impossible. The size of the bathroom only makes things tighter so it makes things more complex.

    A 4 to a skilled plumber is actually more like a 2, a 4 to a total novice can be more like a 6.

    The easier the changes are depends on how much structure and what kind of structure are beneath you and that adds the most variables making it harder to judge.
