where can i find a diabetic sliding scale?

im looking for a sliding scale. i lost the one i used to have and was wondering if there may be one online i could copy.

    less than 150mg/dl ...nothing

    150-200....2 units

    200-250 ... 4 units

    250-300 ... 6 units

    300-350 ...8 units

    350-400 ...10 units

    if more than 400 call the officer who will give 10-14 units and test urine for ketones

    this application is used in inpatient settings (blood sugar every 2 hours)

    Diabetics can eat anything a non-diabetic can eat. You just have to make an adjustment for carbs/sugars. Learn here https://tr.im/mO93k

    It is better for diabetics to eat slow burning carbs: potatoes, peanut butter, vegetables, breads, dairy products, etc. VS. fast burning carbs: candy, sugared soda, cakes, ice cream, anything high in sugar. Just know what your carb/insulin ratios (how much insulin you have to give yourself for the amount of carbs you are intaking) are. Adjust according to the amount of carbs(sugars) you are intaking. And, you should be fine. Natural sugars are better than processed sugars.

    usually your sliding scale is prescribed by your doctor, but it depends which type of diabetes you have.
