This, of course, depends on many factors. In what context are they seeing or talking to eachother? Are you present at these times? What is your relationship, if any, with her ex?

It could be meaningless, or it could be something you need to address. Or...you could just tell her that you are uncomfortable with this. It sounds like the two of you need to communicate.

That depends on the situation. If they broke up quite a while ago and are still good friends, I wouldn't worry about it, especially if she's only telling her when she's all dressed up or something. But if it's on a daily basis for no real reason...I'd talk to her about it.
